All new patients need to fill out the New Patient form and the History form.

These forms can be filled out on your computer and printed or just print and fill out by hand” Fax or bring forms to office.

Insurance

We are Medicare and PPO providers.

The following is a list of most of our contracted PPO Health Insurance Plans
(subject to change without notice):​

    • UNITED HEALTHCARE PPO

If your health insurance plan is not listed, we may still be affiliated with them. Contact your insurance company directly to verify our participation, our tax ID is 33-0185577.

For HMO plans, our office is only contracted with the following Medical Groups (IPA):
